The R.O.S. Personal History Form will help you and all caregivers gather the information needed about your loved one to engage them, communicate with them and plan events for them. It is not who your loved one is today, but who they were their whole life. Ask friends and family who know and care about your loved one to fill in as much as they can for a complete picture of your loved ones preferences, likes, routines and desires.

All data is saved in a secure cloud space that can be accessed by family, friends and caregivers. Allowing people from across the globe to have input into painting a picture of the whole person.
